Compeer Member

Am I eligible to become a Compeer Member?

Any adult in Licking County, currently in mental health therapy (in a day treatment program, mental health clinic, rehabilitation program or with a private practitioner) may request a referral from his/her healthcare provider. Compeer will then make every effort to arrange a match as quickly as possible.

The application process begins when you or your provider contact the Compeer office.

Applicants meet with Compeer Coordinator for a conversation about your interests, recovery and program opportunities. If we have not yet received a completed referral application, we will ask for your written permission to contact your healthcare provider about your health records. You may sign the form Provided Here to allow us to contact your provider for their referral to the program.

We will send your provider an application to complete about your medical history and interests. This information remains confidential. Your provider must confirm that you are engaged in the recovery or treatment process and are a candidate for Compeer.

The forms may be downloaded from this website and faxed, mailed, emailed, or delivered to our MHALC office. Contact Information

Once application is accepted, members immediately become involved with the group activities, if they choose, until and after their eventual match with a special one-to-one friend. Compeer members meet as a group, at least twice a month for social inclusion and wellness activities. They are also contacted my mail and by telephone several times each month. Activities are free of charge and transportation is provided, if necessary.
